Notice of Privacy Practices

Our commitment

STAMP Love, INC is required by law to protect the privacy of your health information, to give you this notice of our privacy practices, and to follow the terms of the notice currently in effect.

How your information may be used and shared

Without asking for your written authorization, we may use or share your health information to:

• Provide, coordinate, and document your treatment.
• Bill for services and obtain payment, including sharing the minimum necessary information with your health plan.
• Run the practice — for example, scheduling, record keeping, and quality review.
• Meet a legal requirement, such as a mandated report of suspected abuse or neglect, a court order, or a public health requirement.
• Reduce a serious and imminent threat to your safety or the safety of another person.

Any other use or disclosure — including most uses of psychotherapy notes, marketing, and any sale of information — requires your written authorization. You may revoke an authorization in writing at any time, except where we have already acted on it.

Your rights

• Ask to see and get a copy of your record.
• Ask us to correct information you believe is wrong or incomplete.
• Ask us to limit what we use or share, and ask us to contact you a certain way or at a certain address.
• Ask us not to share information with your health plan for a service you paid for out of pocket in full.
• Get a list of who we have shared your information with.
• Get a paper copy of this notice at any time.
• Be told promptly if a breach occurs that may have compromised your information.

Telehealth and electronic communication

Sessions may take place by telehealth for people physically located in Delaware. Email and the contact form on this website are not secure channels — please do not send confidential health information through them. We will use a secure method for anything clinical.

Complaints

If you believe your privacy rights have been violated, you may contact the practice at swhittington@stamplove.org or file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/ocr/complaints. You will not be retaliated against for filing a complaint.

Changes to this notice

We may change this notice, and the change will apply to information we already hold as well as information we receive in the future. The current notice is always available on this page and at the office.
